Near-Miss Cards
A phone page where anyone can share a near miss in 4 taps with pictures: what nearly happened, where, what could have happened, and the quick fix. No name field. It makes a picture card you can download, print or share in your team chat.
It helps teams share risks and talk about them. It is not a legal or official incident reporting system. Includes weekly-board.html, a printable 'what we learned this week' board.
